Have people been unsafe in unisex bathrooms?

The only unsafe bathroom I've ever been in is one where the stalls had not been properly secured to the floor, and the whole thing looked like it was on the verge of collapse.

...All the unisex bathrooms either are physically walled off, or have floor to ceiling stalls with the privacy covers on all cracks. (US)
