In newspapers, whenever someone is described as "controversial and outspoken," what is generally meant is "delusional and loudly ignorant."
Also right wing extremist.
Not every time, but enough times that I just assume at this point.
It used to be "excentric" but, as we all know, language is constantly evolving.
